POSTPARTUM CONTRACT
Services of a Postpartum Doula:
A Postpartum Doula works with each family individually to find out their particular needs. The postpartum services that a doula will perform include:
⦁ Breastfeeding and Infant feeding support.
⦁ Help with the emotional/physical recovery after birth.
⦁ Light housekeeping such as baby related dishes and laundry.
⦁ Newborn care such as diapering, bathing, feeding, soothing, comforting, and gentle sleep support.
⦁ Light meal preparation for the family.
⦁ Referrals to local resources such as parenting classes, pediatricians, lactation support, and other parenting support groups.
Postpartum Doula Terms of Practice:
A Postpartum Doula provides evidence-based information on things such as emotional and physical recovery from birth, parent/baby bonding, safe infant sleep practices, infant feeding, soothing, and basic newborn care. A Postpartum Doula is there to offer non-judgmental support to clients in those first days, weeks, and months after bringing home a new baby. A Postpartum Doula is not a medical practitioner, and will not perform any clinical procedures. A Postpartum Doula does not assess for recovery of vaginal tears or cesarean incision recovery of the client, although a Postpartum Doula may provide traditional methods of helping the mother recover from birth, whether cesarean or vaginal and will observe for signs of infection and/or postpartum depression, and educating parents on signs and symptoms regarding these issues. A Postpartum Doula is not a babysitter and does not offer to stay alone in the client's home with the baby. A parent must be at home during all hours of postpartum support. A Postpartum Doula may provide information for the partner to assist in observing signs of infection and/or postpartum depression and/or anxiety.
Confidentiality:
The Doula agrees to maintain strict confidentiality regarding all personal, medical, and sensitive information shared by the client or observed during the course of service, in compliance with the Health Insurance Portability and Accountability Act (HIPAA). The Doula shall not disclose such information to third parties without the client’s explicit written consent, except where required by law, or in cases of concern for the safety and well-being of the client, baby, or others.
Obligations to the Client:
Services listed above are to be provided to [ADD NAME BELOW] (hereinafter known as the Client), at the address of [ADD ADDRESS BELOW]. Kelly Hickman (hereinafter known as the Doula), will be on time for Client's care and stay the entire agreed upon time. If the Doula is unable to come or stay at a scheduled shift, she will inform the Client or a designated representative of their family as soon as she knows she is unavailable and will do her due diligence to either find a backup or reschedule the shift for a later date.

Failure to Provide Support:
The Doula will make every effort to be with the Client at the scheduled times listed below. If the Doula is not able to come due to sickness, or other emergencies, such as a family emergency, or attending the birth of a client, she will inform the Client with as much notice as possible. If Doula needs to leave the Client's home during a shift for any emergency reason or to attend to a birth client, she will provide support to the Client on another day to make up any missed scheduled hours. If Doula is indefinitely unable to provide support, she will help the Client find a permanent replacement within her network of recommendations.
CONCERNING COVID19:
Doula's Responsibilitie

. The Postpartum Doula agrees to immediately notify the Client if the Doula becomes aware of any potential exposure to COVID-19 or develops symptoms consistent with COVID-19.
B
. The Postpartum Doula will not return to work until a negative COVID-19 test result is obtained.
Client's Responsibilities
:
A
. The Client agrees to promptly notify the Postpartum Doula if any member of the Client's household experiences symptoms of COVID-19 or has been exposed to COVID-19.
B
. The Client agrees that the Postpartum Doula will only resume work after the Client has provided confirmation of a negative COVID-19 test result.
Cancellation and Billing
:
A
. If, upon arrival for a scheduled shift, the Postpartum Doula discovers that any member of the Client's household is symptomatic or has been exposed to COVID-19 without prior notification from the Client, the scheduled shift will be immediately canceled.
B
. In such an event, the appointment shall remain billable in full, and the Client shall not be entitled to a refund or rescheduling for the missed appointment. By signing below, both parties acknowledge and agree to abide by these terms to ensure the health and safety of all individuals involved.
Billing:
The client is purchasing
one
4-hr daytime postpartum shift at $65 per hour. The retainer fee for the hours purchased is $260 and is due at the signing of this contract. This retainer fee is final and non-refundable.
Refunds:
The retainer fee is final and non-refundable. This fee is intended to secure the Doula's time and commitment on her calendar to the Client, making herself unavailable to other potential clients during this time, and as such, no reimbursement will be provided if the Client chooses to cancel or back out of the contract at any time. Should the Client decide to terminate this Agreement prior to the commencement of services, the Client acknowledges and agrees that the retainer fee is forfeited. If Doula is indefinitely unable to provide support, only then will a refund be issued.
